The following instructions will provide assistance submitting the agreement and campus form:
- Click Submit a Request on the Receiver District tile
- Complete each field
- My question is about: select Documents, Forms, and Agreements
- Subject and Description: Enter Receiver District Agreement and Campus Activation Form
- Complete additional fields
- Click Add file in the Attachments field to attach your document(s) to the ticket. Documents can be combined into one file or submitted as two separate files.
- Scroll down and Submit
Important Note: Please complete and attach BOTH the Receiver District Agreement and the Campus Activation Form.
