How do I submit the new Receiver District Agreement and Campus Activation Form?

The following instructions will provide assistance submitting the agreement and campus form:

  • Click Submit a Request on the Receiver District tile
  • Complete each field
  • My question is about: select Documents, Forms, and Agreements
  • Subject and Description: Enter Receiver District Agreement and Campus Activation Form
  • Complete additional fields
  • Click Add file in the Attachments field to attach your document(s) to the ticket. Documents can be combined into one file or submitted as two separate files.
  • Scroll down and Submit

Important Note: Please complete and attach BOTH the Receiver District Agreement and the Campus Activation Form.

* If your business office does not have a group email address, please provide complete contact information for the invoice contact.
